The practice may want to get necessary details filled in by the patient. The Custom Clinical Forms feature helps practice users to create the customized forms by themselves for different categories: Patient Demographics, History, Encounter, Health Maintenance, Immunization, Treatment Plan, Facesheet, Chief Complaints, Problem List, Vitals, Medications/Allergies, ROS, Lab Results, Physical Exam, Diagnosis, Review of Result, and Supplementary. When you access any of these categories in the system, the Custom Clinical Form icon is displayed in the menu items on the top right corner. You can use this icon to create the form. You can also edit the CCF form details for various charting elements even after the encounter is closed.
To create a custom clinical form for Facesheet:
1. | On the Facesheet screen, click the Custom Clinical Form icon . |
2. | Select the Template from the drop-down list. |
4. | Design the clinical form and click Save. A script prompt is displayed. |
5. | Enter a File Name and select the Folder Name from the list. |
6. | Click Save. The clinical form is saved for that selected template. Once the form is saved, it is placed in the Document Manager inside the folder that you have selected at the time of saving the form. |
Notes:
• | Similarly, you can create custom clinical forms for various categories mentioned above. E.g., if the custom clinical form is created for encounters, then the screen name appears as “Custom Clinical Form: Encounter.” |
• | Click Delete to delete the form. |
• | Click Print Preview to view the form and print it. |
To view a custom clinical form:
1. | On the Facesheet screen, expand the Encounter Details panel. |
2. | Click the icon for the encounter for which you want to view custom clinical form. The visit details of the patient are displayed. |
3. | Click the Custom Clinical Form link. A list of clinical forms created for various categories is displayed. |
4. | Click any File Name to view a particular form. |
Note: You can also preview and print all the custom clinical forms by selecting the Custom Clinical Form(s) check box on the Encounter note.
