This section helps to add patient's address and contact details. When adding patients address details, enter the Zip code and State/Region and City/Location will auto-populate. In case single zip code assigned to multiple cities, users can select the desired address using the drop-down list.

In case the zip code you have entered is not found, you can configure it using the icon on the fly.

Select the Patient denied/refused to provide email address check box, if the patient does not want to provide e-mail address to the practice. Once the check box is selected, the Email field is grayed out to restrict user to enter e-mail address. Such patients need not be inquired about the e-mail address anytime.

Select the Do not send Patient Statement via E-mail check box, if patient does not want to receive 'Patient Statement' via E-mail.

Select the Exempt from Appointment Reminder Services check box, if patient does not want to receive reminder for the appointment.

The user can tag one or more contact numbers if the patient has intimated them not to contact on specific numbers. Click the icon next to the contact number to restrict the user to call that number. Click the icon to restrict the user to send voice mail on that number. On clicking these icons, they change to the red icons and that indicates that users have restricted to contact these numbers. If you want the users to allow contacting to the patient on these numbers, you can click these icons again and they will turn into green.
